Case Manager (San Joaquin)

POSITION SUMMARY:

The Case Manager will serve and provide support to adults with mild to severe mental health diagnosis. This person should be focused and motivated, while working with therapists and facility program managers to handle client-related issues.

Job Type: Full Time Pay: $35 – $38

Work Location: San Joaquin

RESPONSIBILITIES

  • Complete initial and updated Needs and Service Plans (NSP’s).

  • Meet with each client 1×1 to discuss NSP goals, placement or discharge.

  • Provide individual rehab and case management services to adults. diagnosed with moderate to severe mental illness.

  • Provide a summer of clients for all county meetings.

  • Plan and facilitate weekly group sessions using provided curriculum.

  • Plan fun in-house activities for clients.

  • Transport to scheduled court appointments (as needed)

  • Locate placements for clients

Meet with county case managers and conservators as needed.

QUALIFICATIONs

  • Bachelor’s degree preferred.

  • Must have a minimum of 45 college units with at least 30 units in psychology, social work, nursing or related field

  • Must be at least 21 years of age.

  • At least one (1) year of mental health and psychiatric experience preferred.

  • Ability to work independently and be self-motivated.

  • Excellent interpersonal skills with the ability to effectively work with individuals and groups.

  • Ability to flex work hours as needed.

  • Possess strong interpersonal leadership, analytic, and problem-solving skills.

  • CCL approved FBI and DOJ criminal background clearances.

  • Valid driver’s license.

  • Tuberculosis clearance

  • CPR/First Aid/BLS certification

PHYSICAL REQUIREMENTS

  • Ability to climb stairs multiple times a day and be able to lift and/or carry up to 25 pounds.

  • Ability to sit, rise, kneel, squat, twist, pull bend multiple times a day.

  • Ability to walk with items in hand, grasp and reach as needed.

  • Ability to perform with repetitive maneuvers and manual dexterity.

  • Ability to work in an office environment.
