This person should be focused and motivated, while working with therapists and facility program managers to handle client-related issues.<\/p><p>Job Type: Full Time Pay: $35 &#8211; $38<\/p><p>Work Location: San Joaquin<\/p><h4>RESPONSIBILITIES<\/h4><ul id=\"l1\"><li data-list-text=\"\u2022\"><p>Complete initial and updated Needs and Service Plans (NSP\u2019s).<\/p><\/li><li data-list-text=\"\u2022\"><p>Meet with each client 1&#215;1 to discuss NSP goals, placement or discharge.<\/p><\/li><li data-list-text=\"\u2022\"><p>Provide individual rehab and case management services to adults. diagnosed with moderate to severe mental illness.<\/p><\/li><li data-list-text=\"\u2022\"><p>Provide a summer of clients for all county meetings.<\/p><\/li><li data-list-text=\"\u2022\"><p>Plan and facilitate weekly group sessions using provided curriculum.<\/p><\/li><li data-list-text=\"\u2022\"><p>Plan fun in-house activities for clients.<\/p><\/li><li data-list-text=\"\u2022\"><p>Transport to scheduled court appointments (as needed)<\/p><\/li><li data-list-text=\"\u2022\"><p>Locate placements for clients<\/p><\/li><\/ul><p>Meet with county case managers and conservators as needed.<\/p><h4>QUALIFICATIONs<\/h4><ul id=\"l1\"><li data-list-text=\"\u2022\"><p>Bachelor\u2019s degree preferred.<\/p><\/li><li data-list-text=\"\u2022\"><p>Must have a minimum of 45 college units with at least 30 units in psychology, social work, nursing or related field<\/p><\/li><li data-list-text=\"\u2022\"><p class=\"s3\">Must be at least 21 years of age.<\/p><\/li><li data-list-text=\"\u2022\"><p class=\"s3\">At least one (1) year of mental health and psychiatric experience preferred.<\/p><\/li><li data-list-text=\"\u2022\"><p class=\"s3\">Ability to work independently and be self-motivated.<\/p><\/li><li data-list-text=\"\u2022\"><p class=\"s3\">Excellent interpersonal skills with the ability to effectively work with individuals and groups.<\/p><\/li><li data-list-text=\"\u2022\"><p>Ability to flex work hours as needed.<\/p><\/li><li data-list-text=\"\u2022\"><p>Possess strong interpersonal leadership, analytic, and problem-solving skills.<\/p><\/li><li data-list-text=\"\u2022\"><p class=\"s3\">CCL approved FBI and DOJ criminal background clearances.<\/p><\/li><li data-list-text=\"\u2022\"><p>Valid driver\u2019s license.<\/p><\/li><li data-list-text=\"\u2022\"><p>Tuberculosis clearance<\/p><\/li><li data-list-text=\"\u2022\"><p><span style=\"font-family: -apple-system, BlinkMacSystemFont, 'Segoe UI', Roboto, 'Helvetica Neue', Arial, 'Noto Sans', sans-serif, 'Apple Color Emoji', 'Segoe UI Emoji', 'Segoe UI Symbol', 'Noto Color Emoji';\">CPR\/First Aid\/BLS
